Before Your Visit
Please complete any required patient forms prior to your appointment and bring your insurance card, photo ID, and a current list of medications. If you are a new patient, we recommend arriving a few minutes early to complete check-in.
During Your Appointment
Check-in online or in the clinic with the front desk staff. You will be directed back to an exam room. Staff will come in to greet you and get your vital signs and check you in. Then the provider will enter and the visit will start.
After Your Visit
At the conclusion of your appointment, our staff will assist with scheduling follow-up visits, referrals, lab work, or prescription needs as necessary.

Please call ahead if you need to cancel or reschedule your appointment. We ask patients to arrive on time so we can provide the best care experience for everyone.
